What to Know Before Your First Appointment


During the evaluation we will discuss your medical history along with pertinent need to knows about medical marijuana. Please note we want to be equipped to qualify you. In order to help us help you, we will need for you to bring medical records to assist with evaluation and qualifications. We strongly recommend providing this information prior to your appointment so your practitioner will have ample time to review.


Accepted records include:


  • Records/notes from doctor’s visits
  • Discharge summaries from hospital
  • Current prescription medications or history of medication
  • Proof of prior medical marijuana card if you resided in another state
  • Any other notes from medical professionals

Book Appointment

Schedule an in-office appointment to meet with your medical cannabis practitioner. Please be prepared to provide some basic medical history.

Talk to a Provider

Complete an evaluation with a medical cannabis practitioner which will determine if you qualify to receive a written certification.

Receive Medical Card

Apply for your KY medical cannabis card. If approved, you will be able to download your registry ID card from the Office of Medical Cannabis.

All appointments must be paid in full at time of booking. However, if you decide to cancel the appointment, you will be refunded as long as notification is provided at least 48 hours before your appointment. We proudly offer a 10% discount to veterans!

Initial Medical Cannabis Evaluation


Initial Medical Cannabis Evaluations are for patients who want to qualify for a medical cannabis card. Please see the FAQ section for details on qualifications. In general this is for patients that have had a chronic, ongoing condition for which they have already been seeing a provider or specialist for. While we will not be diagnosing conditions in the office due to the chronic nature of a lot of the conditions, we have arranged for telehealth referrals to a qualified mental health professional that we will work along side of for those who feel like they may have post traumatic stress disorder (PTSD) that is undiagnosed. We will also be able to assist patients with setting this up while at the office. 


Please note: There is a $50 consultation fee charged to hold the appointment. This consultation fee is credited towards the $125 charge. If you do not qualify for a medical cannabis card during your evaluation, you will not pay the remaining $75 for the appointment.


Follow-up Consultation


Follow up appointments will be available for patient’s who desire a more personalized approach to their medical cannabis journey. While cannabis can be very beneficial in many ways, guidance is helpful so that your experience can be a positive one especially if you are brand new to cannabis. Dosing cannabis can be imprecise initially and complex due to the varieties and forms that can produce various effects. Please let us support you!

  • What are the medical qualifications for considerations for a KY medical cannabis card?

    At this time:


    • Any type or form of cancer
    • Chronic or severe pain
    • Epilepsy or other intractable seizure disorder
    • Multiple sclerosis, muscle spasms, or spasticity
    • Chronic nausea or cyclical vomiting syndrome 
    • Post-traumatic stress disorder

    There was a letter sent to Kentucky Legislative Leadership recommending expansion of this list. We hope to see more conditions being added to the list and will update as we are made aware.

  • What is the qualification criteria for Kentucky residents?

    • You must be a resident of KY
    • You must have a medical qualification that is diagnosed
    • You may NOT have any disqualifying felony offense convictions
  • When can I submit my application for a KY medical cannabis card?

    The Patient and Caregiver Registry Portal will be available by January 1, 2025.

  • Is there an application fee?

    Yes, the application fee paid to the Kentucky Medical Cannabis Office is $25 once you submit your application.

  • What will I need to have available to submit the application?

    Full name, address, phone number, email, DOB, SSN, and DL number. Additionally you must sign a notarized signature page that affirms you will not divert medical cannabis to unauthorized persons. 

  • How long is my written certification for medical cannabis valid?

    This expires after 60 days following the date it was issued by your medical cannabis practitioner and will not be accepted if expired. Should this expire, you would need to certify again so we recommend submitting application in a timely fashion to avoid this from occurring!

  • Do I have to renew my KY medical cannabis card?

    Renewal is required annually.

  • Does insurance cover my medical cannabis evaluation with Bluegrass Oasis?

    No, insurance does not cover medical cannabis evaluations.

  • Can I complete my appointment via Telehealth?

    Kentucky requires a bona-fide relationship between patient and provider and therefore requires your first visit to be in the office. However, the good news is recertifications can take place via Telehealth! 

  • Are there age restrictions for medical cannabis?

    Qualified patients under 18 years of age may only use medical cannabis with the assistance of a caregiver who is the patient’s parent or legal guardian. Please note Bluegrass Oasis will not be taking patients who are under the age of 18 years old regardless of consent.

  • How will I obtain my medical cannabis once approved?

    This will be obtained via licensed and regulated dispensaries which will be located throughout KY. 

  • May I grow my own cannabis?

    No, in order to grow cannabis in KY you would need to have a cannabis business license. 

  • How can I learn more about KY’s medial cannabis program?
